Color Doppler Ultrasonography of Thyoid Masses: Differentiation of Papillary Carcinoma from Adenomatous Goiter




Abstract
We evaluated the value of the color Doppler ultrasonography in the evaluation of thyroid masses to differentiate malignant from benign tumors.
Color Doppler ultrasonography was done consecutively in 88 patients with thyroid diseases. Among them, we studied fourty-six cases included 27 adenomatous goiters and 19 papillary carcinomas that were diagnosed by needle aspiration(24 patients)
and
surgery(22 patients). The analysis of color Doppler ultrasonography was performed by inspecting the color signals in and around the tumors. And the color signals were graded from 0 to III according to the degree of vascularity in the internal
blood
flow
that represented tumor vessels and marginal blood flow that represented feeding vessels or sweeping normal vessels.
Grade I internal color signals were found in 11 cases(41%) of 27 adenomatous goiters and grade III in 10 cases(53%) of 19 papillary carcinomas. Grade II marginal color signals were noted in 12 cases(44%) of adenomatous goiters and grade I in 10
cases(53%) of papillary carcinomas. The relationship between the degree of internal color signals and malignancy of thyroid masses was statistically significant(P<0.1). However, the grade of marginal color signals was not correlated with the
malignancy
of masses. The results of thyroid function test and radionuclide scanning were not correlated with the degree of internal color signals in adenomatous goiter and papillary carcinoma.
In conclusion, although the increased internal color signals of thyroid masses were slightly overlapped between adenomatous goiter and papillary carcinoma. Increased internal color signals were very helpful in differentiating the malignant from
the
benign tumors.
